This is a CONFIDENTIAL evaluation of how you think the class went this
semester.  You don't have to include your name!  I would like our
opinion of the class and if you have any suggestions for future
offerings of the class so we can continue to make it better for
others.

You can use this form, or type something and print it to keep it even
more confidential.  Give evaluations to Debbie in the ECE office, or
slip evaluations under my door (MEB 4506).
