ECE 3970 Undergrad Research Request

ECE 3980 Independent Project Request

Please meet with an ECE faculty or approved advisor to complete appropriate form. Then submit it to your EE or CE academic advisor or the ECE Main Office (MEB 2110) to be enrolled in credits.
ECE 3990 Co-Op/Internship Request Students must meet with an EE or CE academic advisor to review their eligibility before submitting the request form. Students must complete form before being enrolled in credits.
Exception to Policy
Major Status Application Please meet with an ECE faculty or approved advisor to complete the form.
Transfer Petition This links to the transfer petition request form for Electrical and Computer Engineering Courses. If you need to request transfer evaluation for a class in a different department, you need to go to that department’s specific evaluation form which can be found at this link.
BS/MS or 4+1 Degree Plan To be considered a part of a Bachelor’s to Master’s program students must first meet with the Graduate Student Coordinator to review their eligibility and complete the degree plan. Students will be unable to register for or request graduate-level coursework until they have completed this step. See Bachelor’s to Master’s page for more information.
Apply for Graduate Status For students wishing to apply to the ECE Master’s or PhD degree programs. All BS/MS and 4+1 students will need to apply for graduate status their senior year. For questions, please meet with the Graduate Student Coordinator.
Apply for Undergrad Graduation All undergraduate students must apply in advance for graduation to receive a degree from the University of Utah.  Before applying to graduate, students should meet with their academic advisor and complete the following checklist.

General

Form Description & Instructions
Transfer Credit Forms Students should transfer eligible credits immediately upon entering the graduate program to ensure that credits have not expired and that a student can properly prepare a plan for their degree.
Leave of Absence Domestic graduate students may request a leave of absence to postpone their studies for up to one year after their first graduate-level term. International Students on a F1 or J1 visa taking a vacation semester or a medically necessary reduced course load of zero credits must obtain permission from the ISSS Office.
Defense Announcement Form Once MS Thesis and PhD students have their defense, we will post the event to our department calendar and advertise it to our faculty, students, and local industry professionals.
Application for Graduate Graduation All graduate students must apply in advance for graduation to receive a degree from the University of Utah. Before applying to graduate, students should meet with their academic advisor and review the degree requirements set by the Graduate School.

MS

Form Description & Instructions
Graduate Degree Plan For planning out your degree or tuition reimbursement (i.e. PAQ program)
Project Credit Request Complete form and return to the Graduate Student Coordinator to be enrolled in ECE 6950 MS Special Study Credits (For the MS Project Option). For all steps and information please review the MS Project page.
Project Final Verification When their MS project is completed, students need to submit this form to the Graduate Student Coordinator before the end of their final semester.
Supervisory Committee This form is primarily intended for MS Thesis students. MS students completing the coursework or project options will be assigned the default committee members from the applicable area of emphasis.

PhD

Form Description & Instructions
Supervisory Committee Students may complete this form with signatures, or send list of committee members to the Graduate Student Coordinator to be entered into the Grad Tracking system. For committee members outside of the University of Utah the following will also need to be provided:

  • CV (updated within the last 3 years)
  • Justification Letter – The student should submit a paragraph explaining why the committee member was chosen to search on the student’s committee and how they are qualified to assist with the student’s research. The Graduate Student Coordinator will formalize the letter with letterhead and DGS signature.
Program of Study Complete form and return to the Graduate Student Coordinator.
